Birth Certificate Applications

Birth certificates are very important nowadays because every time you are going to look or to apply something one of the requirements that they will ask you is to present your birth certificate. This document is the record of your information from the day that you were born. So let us see how these birth certificates created.

Here are the sample birth certificate applications that you're going to fill up from the day that your baby was born.

Birth Certificate Applications
  1. The legal name of the baby. First name, middle name, and the last name. If the baby has suffix name write it down.
  2. The birth certificate applications will ask you if you want to have your child a social security number. You're going to check the two boxes of yes or no.
  3. The current legal name of the mother.
  4. The maiden name of the mother. Meaning she's going to write the last name that she was born.
  5. The date that the mother was born.
  6. The social security number of the mother.
  7. Write the place that the mother was born if it state or foreign country.
  8. The official name of the city that the mother is residing.
  9. The birth certificate applications will going to ask you if the address is city limits then you're going to mark check on the boxes yes or no.
  10.  Your home telephone number.
  11.  The address of the mother for she will going to receive mails. And if she have entered the same information on the number 8 above then she will have to enter only the zip code.
  12. The birth certificate applications will ask the mother is she is married. Then she is going to mark check from the boxes of yes or no. There is a note that if the mother is not married then the father's name are only be shown on the birth certificate if they are  acknowledge paternity.
  13. Father's name.
  14. The date that the father was born.
  15. The social security number of the father.
  16. The place that the father was born if it state or foreign country.
  17. The mailing address of the father.
  18. The father's home telephone number.
  19. The mother's race and ethnicity. Then the mother will have to check her race from those boxes indicated and the Hispanic origin.
  20. The father's race and ethnicity. Then the father will have to check his race from those boxes indicated and the Hispanic origin.
  21. The education of both parents completed. Specify the highest grade finished.
  22. The birth certificate applications will ask the parents about if they worked for the past years. Then they will have to check the boxes of yes or no.
  23. Then if they checked yes. Write down what occupation.
  24. Then they are going to indicate the information of their work. Like the Business or industry of that work, the name of the employer, and the address of it.
  25. If other than mother, the informant will write his or her name.
  26. The relationship of the informant to the child.
  27. Signature of mother or the informant that she certifies that the information written is correct.
  28. The name of the hospital representative that will review the information of the mother or the informant.
  29. The date of the review.

Then on the third sheet of the birth certificate applications is the instruction for the parents for the birth certificate and the important to remember box at the lower part of the paper.
